Empowering patients to breathe easier

The LinkMyCare – COPD program was rolled out across South Eastern NSW, with the first group of general practices enrolling patients. Funded by NSW Health and co-designed by the South Eastern NSW Strategic Partnership – including Southern NSW and Illawarra Shoalhaven LHDs and COORDINARE – the program is already making a meaningful difference in the lives of people living with Chronic Obstructive Pulmonary Disease (COPD).

Through personalised COPD Action Plans, patients like Yvonne and Brian from Cooma are gaining the tools and confidence to manage their condition at home, reducing hospital visits and improving their quality of life.

Yvonne’s story: Confidence through clarity

After frequent hospitalisations and uncertainty about when to seek help, Yvonne now feels more in control. 

Since her GP and a LinkMyCare – COPD nurse, helped write a COPD Action Plan, she has felt more confident in her ability to manage her symptoms and has been able to avoid going to hospital.

Brian’s story: Peace of mind after hospitalisation

Following a hospital stay due to severe breathing difficulties, Brian also worked with his care team to create a COPD Action Plan.

Since starting the plan, Brian hasn’t had another attack and says it’s made a huge difference to his peace of mind.
