Implemented personal logins on both HealthPathways sites, plus Smart Search (AI-powered tool to improve search functionality).

Showcased HealthPathways at the Shoalhaven Medical Association conference.

Released our new Palliative Approach Framework and accompanying quick reference guide.

Received funding to deliver after hours mobile primary care service to residential aged care facilities and reduce avoidable hospital visits.

Saw the Southern NSW Voluntary Assisted Dying Program team awarded Rural/Remote Multidisciplinary Collaboration of the Year.

Celebrated the Southern NSW LHDs ‘Escape the Vape’ campaign winning an Agency for Clinical Innovation Award

Placed clinical nurse consultants into the Local Health Districts as part of the LinkMyCare COPD program.

Had alcohol and other drug targeted enhancement service grant approved, offering further contracts to Directions Health, Salvation Army, and Clean Slate for telehealth withdrawal programs.

Partnered with other PHNs to develop a Residential Aged Care Home (RACH) Telehealth Training Program

Invited health providers to apply for aged care virtual care grants to strengthen RACHs connections with external care providers.

Established the Integrated Care and Commissioning stakeholder advisory group bringing together 18 aged care, disability, community, employment and training organisations to improve access in regional and rural areas.

Supported and hosted many residential aged care staff forums across our region. Also connected with providers, seniors, caregivers and community at the Illawarra Aged Care Expo.

Empowering the care finder workforce

Since July 2024, COORDINARE has strengthened the care finder workforce through quarterly Communities of Practice (CoPs). These face-to-face forums offer a safe, structured space for staff and managers to reflect, share insights, and solve challenges together.
