HealthPathways: Strengthening collaboration and clinical support

The Illawarra Shoalhaven and Southern NSW HealthPathways teams have worked closely to align strategic priorities and streamline processes. This collaborative approach has enhanced regional consistency and improved the delivery of coordinated, efficient, and patient-centred care across South Eastern NSW.

Key achievements throughout the year included:

Developed women’s health and menopause pathways, supported by stakeholder engagement and a multidisciplinary education event.

Strengthened GP referrals and internal collaboration in Southern NSW to improve communication with local GPs.

Partnered with aged care homes to promote outbreak management, immunisation, and relevant clinical pathways.

Prepared clinical and referral pathways for the National Lung Cancer Screening Program.

Finalised dementia and aged care pathways, with ongoing updates to support evolving care needs.

Gathered GP feedback in the Illawarra Shoalhaven to improve dementia pathways and system navigation.

These efforts ensure health professionals have access to up-to-date, localised resources to support patient-centred care.

Dementia resource development

In partnership with key stakeholders, COORDINARE launched a dedicated dementia support website offering region-specific information for individuals living with dementia, their carers, and health professionals.

The site includes dementia-specific clinical pathways for Illawarra Shoalhaven and Southern NSW, and a one-page guide to enhance care delivery. Printable resources were also developed to share with patients and caregivers, as well as tailored versions for Aboriginal communities.

Ongoing collaboration with Dementia Australia, Dementia Training Australia, the University of Wollongong, and LHDs continues through roadshows, forums, and engagement activities – strengthening support for people living with dementia across South Eastern NSW.
