Supporting better COPD care in the community

COORDINARE, in partnership with the Illawarra Shoalhaven and Southern NSW LHDs, is advancing a multi-disciplinary approach to managing chronic obstructive pulmonary disease (COPD) through the LinkMyCare – COPD program.

This initiative funds a range of providers to deliver community-based care, helping reduce hospital visits and improve coordination between hospitals and general practices.

Dr Lisa Opie, practice owner at Thrive GP in Crookwell, says the program has already delivered clear benefits for both patients and the practice.

The program provides funding for practice nurse resourcing, to assist GPs to provide monitoring, education and quality services to patients with COPD. This improves self-management and reduces the need for patients to attend hospital.
