Ten years of building healthier communities

In 2025, COORDINARE – South Eastern NSW PHN proudly celebrates a decade of improving health and wellbeing across one of NSW’s largest rural and regional populations. 

Since being established in July 2015 with Australian Government funding, we have worked in partnership with communities, healthcare providers, and service organisations to coordinate care and support those most at risk of poor health outcomes. 

This milestone is not only a reflection of our organisational journey – it’s a tribute to the people who have shaped it.
